T-Score vs Z-Score: What the Difference Actually Means for You
Your DEXA report lists both a T-score and a Z-score for each measurement site. They look similar — both express your bone density as a standard deviation from a reference — but they answer different questions and apply to different situations.
The T-score: vs a healthy young adult
The T-score compares your bone density to the average peak bone density of a healthy 30-year-old of the same sex and ethnicity. It is expressed in standard deviations:
- T-score ≥ -1.0: normal
- T-score -1.0 to -2.5: osteopenia
- T-score ≤ -2.5: osteoporosis
- T-score ≤ -2.5 plus fragility fracture: severe osteoporosis
For post-menopausal women and men over 50, T-score is the primary number that guides treatment decisions.
The Z-score: vs people your age
The Z-score compares you to the average bone density of people your own age, sex and ethnicity. It is more relevant for:
- Premenopausal women (under 50)
- Men under 50
- Anyone losing bone faster than expected for their age
- Cases where secondary causes (steroids, hyperthyroidism, coeliac, low BMI) are suspected
A Z-score of -2.0 or lower is a flag worth investigating with bloodwork. It means you have lost more bone than is typical for your age cohort.
What the typical post-menopausal pattern looks like
Z-score around 0 (normal for age) with a T-score in the osteoporosis range. This is what we usually see: a woman’s bone density is roughly average for her age but has fallen past the protective T-score threshold simply because she has lived past the protection of estrogen.
This is not a sign of an unusual pathology — it is the normal post-menopausal trajectory and the reason most osteoporosis is treated in this demographic.
The atypical pattern that warrants more workup
A young Z-score significantly below -2.0 suggests something is accelerating bone loss beyond age expectations. Possibilities to investigate:
- Hyperparathyroidism
- Hyperthyroidism (over-treated or endogenous)
- Long-term steroid use
- Coeliac disease (causes calcium malabsorption)
- Low body weight or eating disorder history
- Premature menopause
- Genetic conditions affecting collagen
If your Z-score is unusual, ask your physician for a secondary-cause workup, not just bisphosphonate prescribing.

Can my T-score and Z-score disagree?
They always do, by definition — they use different reference populations. The disagreement is informative, not a problem.
Why does my spine T-score differ from my hip T-score?
Bone density varies by site. Spine is often artificially inflated by osteoarthritis or calcified vessels; hip is more reliable for prediction. Most guidelines use the lower of the two for treatment decisions.
Should I track Z-score over time?
Usually no. Z-score changes are dominated by the changing reference cohort as you age. T-score change over time is the more useful tracking metric.
