Strontium for Bone Density: Is It Safe?

Strontium is a naturally occurring mineral — a close chemical neighbor to calcium — that has attracted significant interest in osteoporosis treatment. Its story is more complicated than most bone supplements, involving both impressive clinical results and real safety concerns that limited its medical use. Here’s what you need to know.
Strontium Ranelate: The Prescription Version
Strontium ranelate was developed as a prescription medication for osteoporosis and was widely used in Europe for over a decade. Clinical trials showed significant improvements in bone mineral density and meaningful reductions in vertebral and hip fractures — results that were genuinely impressive. However, it was later restricted and then largely withdrawn from most markets due to an increased risk of serious cardiovascular events (heart attack, stroke) and skin reactions in some patients. It remains available in a few countries under strict prescribing guidelines for severe osteoporosis.
Strontium Citrate: The OTC Alternative
Strontium citrate is available as an over-the-counter supplement in many countries. It’s sold at much lower doses than the prescription ranelate form. Proponents argue it provides bone benefits similar to the prescription version without the same risks, because it uses a different salt form and lower dosing.
The honest assessment: the evidence for strontium citrate specifically is much weaker than for ranelate. Most of the clinical data comes from the prescription form at much higher doses. Whether OTC strontium citrate provides meaningful bone benefits at supplemental doses is not clearly established.
There’s also a measurement concern: strontium is absorbed into bone tissue and appears on DEXA scans in a way that can artificially inflate T-score readings — because strontium is denser than calcium. Some of the apparent bone density “improvement” seen with strontium may reflect the mineral’s presence in bone rather than true structural improvement.
Should You Consider It?
For most people, strontium is not a first-line consideration. The foundational supplements — calcium, vitamin D3, K2, magnesium — have a stronger and cleaner evidence base. If you have established osteoporosis and are interested in strontium, this is a conversation to have with your doctor, who can assess your cardiovascular risk and weigh the potential benefits appropriately.
